ACHILLES TENDON RUPTURE Accelerated Functional Rehabilitation Protocol (see below)
This is intended to be used by medical professionals so you need to know what these exercises are before doing them! I’m not a medical doctor or physiotherapist so please be careful - I have a physiology degree but not much use now as can’t remember any of it.
Does anyone know what “Active plantar and dorsi flexion to neutral, inversion /eversion below neutral” is? I’m going to ask my friend’s husband who is a PT in Canada so will report back.
0 – 2 WEEKS
Aircast boot with 2 cm heel lift
NWB with crutches
2 – 6 WEEKS
Aircast boot with 2 cm heel lift
Protected weight‐bearing with crutches as required
Active plantar and dorsi flexion to neutral, inversion /eversion
below neutral
Modalities to control swelling
Knee/ hip exercises as appropriate
NWB fitness/cardio work
Hydrotherapy (within motion and weight‐bearing limitations)
6 – 8 WEEKS
Aircast boot
D/C heel lift
WBAT
Dorsiflexion stretching, slowly
Graduated resistance exercises (OKC, CKC, functional)
Proprioceptive and gait retraining
Modalities as indicated
Fitness/cardio to include WBAT
Hydrotherapy
8 – 12 WEEKS
Wean off boot
Return to crutches/cane as necessary; then wean off
Continue to progress ROM, strength, proprioception
>12 WEEKS
Continue to progress ROM, strength, proprioception
Retrain strength, power, endurance
Increase dynamic WB exercise, include plyometric training
Sport specific retaining

Active= just the foot working with no help or resistance.
Below neutral= keeping your foot pointed, neutral is a 90 degree angle between foot and leg.
Inversion and eversion= working the outside and inside of the foot down.
Dorsi= foot up
Plantar= foot down
I love these exercises and also rotate clockwise and anticlockwise, it is incredible to see how quickly the body responds to doing this regularly.
Everything is progressive, so patience and doing this a few times a day pays of big time!

This protocol is exactly the same as those that my OS prescribed. I’ve witnessed improvement in ROM by going through these exercises a few times a day over the past week or so.
Knee/hip exercises that my PT prescribed are done with the boot on to to keep them from going bad from imbalance of the legs I believe.
With BOOT ON:
1) While sitting down, lift the boot to straighten the leg and hold for a few seconds. 10 reps @ 3 times a day.
2) Do a sitting march. While sitting down, lift knees alternating each side holding the up position for a couple of seconds. 10 reps @ 3 times a day.
3) While lying down on your back, bend the bad leg as close to your buttocks as you can and hold for a few seconds. 10 reps @ 3 times a day.
Although it wasn’t part of my prescribed exercise, I totally agree with herewegoagain that rotation of the foot clockwise and counter-clockwise is great exercise. I’d do it after you’ve done the plantar flex, inversion, eversion exercises. So that the tendons are warmed up.
Also, my PT mentioned that just as a precaution, I should make sure that my tendon is warm when I do the ankle exercises so as not to put stress on the tendon when it is cold. But I think you’ll find that because your bad leg is in a boot, it’s generally kept warm most of the time.

I think that version is identical to the one I posted at bit.ly/UWOProtocol, if you want to save some keystrokes. That’s the version the UWO=Fowler Kennedy folks faxed to my OS in Summer 2009.
(It seems BETTER than the version that’s linked from the end of the published 2010 UWO study (Willets - or is it Willits? - et al)!)
